Waiver & Release

To participate in any paid event with Jason Fazio Photography, you must read, sign, and return the Waiver and Release Form linked below.

Two Options Available:

  • *Download, Sign, and Email It before the event (preferred)

  • Print, Sign, and Hand-Deliver during event

*Note: The Contact Form will not have an option to add an attachment. No worries. Simply use the contact form to let me know that you have a waiver form to return. When I reply, you will then be able to add an attachment to a normal email.
